A1C estimates average blood sugar over about two to three months. It is commonly used to screen for prediabetes and diabetes.

A high fasting insulin level may suggest the body is working harder than it should to maintain blood sugar. Some clinicians also use calculations such as HOMA-IR, which combines fasting glucose and fasting insulin, though it is not always used in standard primary care.


Cholesterol and triglycerides show how fats are moving through the blood

Why waist circumference matters


Waist circumference is simple, low-cost, and surprisingly useful. It gives a rough estimate of abdominal fat, especially visceral fat. Visceral fat sits deeper in the abdomen around organs. Higher amounts are linked with insulin resistance, higher triglycerides, fatty liver, blood pressure changes, and inflammation.

Waist circumference should not be used to shame or judge. It is one marker. Its value comes from tracking risk and change, not assigning worth.


Blood pressure is a metabolic health signal

Liver enzymes can point to fatty liver risk

  • Fasting insulin, when appropriate

  • Blood pressure readings

  • Waist circumference

  • Weight trend, when relevant

  • Liver enzymes

  • Kidney function

  • Family history

  • Sleep, activity, alcohol use, smoking, and medication history


Trends matter more than a single number. A fasting glucose of 98 mg/dL may be labeled normal, but if it rose from 82 over a few years and triglycerides also climbed, that pattern deserves attention. The same is true for blood pressure that moves from 112/70 to 128/82.
